Kratom and alcohol — do not combine.
Direct answer
Combining kratom with alcohol is one of the most consistently dangerous kratom drug interactions. Both are central nervous system depressants, both are hepatotoxic, and kratom inhibits the CYP enzymes that clear alcohol metabolites. If you drink, skip kratom that day — minimum 6–8 hours between any drink and any kratom dose.
Why the combination is dangerous
- Additive CNS depression. Kratom at higher doses acts on mu-opioid receptors; alcohol acts on GABA and NMDA. Both slow respiration. Combined, the risk of respiratory suppression during sleep is materially elevated.
- Hepatotoxicity stacking. Both substances burden the liver. The combination is associated with elevated liver enzymes and, in chronic users, progression to hepatic injury.
- CYP3A4 inhibition. Kratom inhibits CYP3A4, which slows alcohol clearance. Blood alcohol stays high longer.
- Impaired judgment + kratom wobbles. Kratom at high doses produces "wobbles" — nystagmus, dizziness, nausea. Alcohol compounds this dramatically.
- Nausea and vomiting + sedation = aspiration risk. Sedated vomiting can cause lethal aspiration pneumonia.
Documented harm
Published case reports link kratom + alcohol (usually with additional substances) to:
- Acute hepatic injury requiring hospitalization
- Fatal respiratory depression during sleep
- Car crashes under unexpected combined impairment
- Emergency-room visits for severe vomiting and dehydration

- Do not add more of either. Stop both substances immediately.
- Hydrate with electrolytes.
- Have someone stay with you for at least 4 hours. Do not sleep alone.
- Signs of emergency: slow or shallow breathing, blue lips, unresponsiveness, chest pain, seizure. Call 911.

Using kratom instead of alcohol

FAQ
How long should I wait between alcohol and kratom?
At minimum, 6–8 hours for a single drink to clear, longer for heavier drinking. Liver metabolism is the limiting factor. If you are going to drink in the evening, skip kratom that day entirely.
Does kratom cause hangovers?
Mild next-day wobbles, dehydration, and GI discomfort at higher doses — but nothing like alcohol hangovers. Adding alcohol to kratom stacks both.
Can kratom help with alcohol withdrawal?
Some users report using kratom to manage alcohol cravings and mild withdrawal symptoms. Alcohol withdrawal is medically dangerous (seizures, delirium tremens). Do not self-treat. Seek medical care via SAMHSA 1-800-662-4357.
Can I drink the morning after kratom?
Not recommended. Kratom alkaloids have a longer half-life than a few hours, and residual kratom + fresh alcohol produces the same interaction risk as taking both simultaneously.
Are there any documented deaths from kratom + alcohol?
Yes. Case reports exist of fatal polysubstance events involving kratom and alcohol, typically also with other CNS depressants (benzodiazepines, opioids, fentanyl contamination). Pure kratom-only or alcohol-only deaths are rare; the combination is well documented.
